Gastric Bypass in Istanbul, Turkey: Roux-en-Y or Mini Bypass? | New City Clinic

If you have decided on a gastric bypass in Turkey, the next question is often which bypass. Surgeons in Istanbul perform both the standard Roux-en-Y gastric bypass and the mini gastric bypass, also called the one-anastomosis gastric bypass (OAGB). They work in a similar way but are not interchangeable, and reflux is often what decides between them. New City Clinic coordinates bariatric surgery with general surgeons at partner hospitals in Istanbul; this guide explains the differences for patients from the UK, Ireland, the US and Australia.

Who can have a gastric bypass?

A BMI of 35 or more is considered suitable for weight-loss surgery. Between a BMI of 30 and 35, an accompanying disease such as type 2 diabetes is required. The BMI is a starting point: the surgeon makes the final decision after examining you and reviewing your tests. A bypass also requires lifelong vitamin supplements and regular blood tests, so commitment to follow-up is part of being a good candidate.

The standard Roux-en-Y bypass

A small stomach pouch is created and connected directly to a lower part of the small intestine, so food bypasses most of the stomach and the first part of the intestine. A second connection rejoins the digestive juices further down; that is why it has two connections. The operation usually takes two to three hours. Portions become smaller and the hormonal changes often improve blood sugar control. It is usually preferred when reflux is significant.

The mini (one-anastomosis) bypass

In a mini gastric bypass a long, narrow stomach tube is connected to the small intestine with a single connection instead of two, so the operation is usually shorter, about one to two hours. It is still a major bypass procedure with the same lifelong follow-up. Bile reflux is a possible issue, which is why patient selection matters; it is often considered at a higher BMI when the surgeon judges it appropriate.

Reflux: the deciding question

If you already have significant heartburn or reflux, tell the surgeon and send any gastroscopy reports. With significant reflux a standard Roux-en-Y bypass is usually the better fit, because a sleeve can make reflux worse and a mini bypass carries a risk of bile reflux. A gastroscopy before surgery helps the surgeon see the state of the stomach and plan accordingly.

Diabetes and other conditions

In many patients with type 2 diabetes, blood sugar control improves considerably after a bypass; the effect varies, and your doctor at home adjusts your medication – never change it on your own. High blood pressure and sleep apnoea can also improve with weight loss. Bring your recent blood tests, medication list and any specialist letters for the surgeon’s review.

Vitamins and long-term follow-up

After either bypass, iron, vitamin B12, calcium, vitamin D and other nutrients are absorbed less well, so supplements are taken for life and blood tests are checked regularly. Sweet foods can cause dumping – dizziness, sweating and cramps shortly after eating – which is avoided by limiting sugar. Your GP should know about the operation, because long-term blood tests are usually arranged at home.

Your stay in Istanbul and recovery

For both types of bypass, plan about 6 to 8 days in Istanbul with two or three nights in hospital. You walk and sip water from the first day. The diet then moves in stages from liquids to purées, soft food and finally small solid meals over the first weeks. Desk work usually resumes after about two weeks and intense exercise after about six weeks, with the surgeon’s approval. Most of the weight loss happens over the first 12–18 months; results vary and no figure can be guaranteed.

How a bypass compares with a sleeve

A gastric sleeve removes about three quarters of the stomach without changing the route through the intestine; it is shorter and does not need a bypass’s malabsorption follow-up, but it can worsen reflux. Our comparison of gastric sleeve and gastric bypass covers that choice in detail; this guide is for patients for whom a bypass is already on the table.

Travelling and the price

Direct flights to Istanbul run from London, Manchester, Birmingham, Edinburgh and Dublin, at around four hours, and from several major US cities; from Australia the journey involves a connection. Walk during the flight and sip water regularly. A gastric bypass is quoted individually after the surgeon’s review of your BMI, health and tests; the written quote lists the hospital, surgeon, nights in hospital, hotel and transfers. Flights are not included.
